At my last job, we ran a helpline for disabled people. Another organization decided to put an article about it in their newsletter, but didn't let us check it before it went out. They had transposed the last 2 digits of the phone number, so instead of callers reaching our helpline, they got a very sexy voice saying something along the lines of "Are you looking for a good time?" Needless to say, my boss wasn't very happy with the other organization!
