An itinerary for what? For a meeting, for travel, both??? There are so many things you can have an itinerary for that without knowing what type you are looking for it's hard to say. Can you give any more specifics?

Mine is like a table on word with the following:
One distributed to relevant staff: Date Flight/Train details (Mainly flight no, departure and arrival times in respective cities/countries) Hotel(s) details:(inc address, tel/fax no) Meetings scheduled (tiem and company visited) Other info (such as phone number my boss can be contacted on)
One for my boss: Date Flight/Train/Hire car info Hotel details (inc address, tel/fax no) Meetings scheduled (time and company with addresses) Other info (anything else relevant to that trip)
I lay it out with the dates on the left hand side (vertical) and all the headers as above horizontal
