While it wouldn't set off sufficient alarm bells for me to not apply, it would be something I would ask about during an interview.

Likely scenarios:

1) Person hired, found a better job or hated existing job, quit.
2) Person hired, bad fit, fired or mutual decision to terminate
3) Job frozen due to budget constraints, pulled, now being reoffered.

I suppose I could think of other scenarios, but I think these are most likely. The problem is there is no way for us to know from the outside if the person sucked as an employee or the boss/company sucked as an employer.

Equally importantly, sometimes it just comes down to working style. I have worked for a number of bosses that other admins couldn't work for and have had few problems.  So the only thing that really matters would be how well YOU would fit the boss.
