I use a word that has great personal meaning to me, but wouldn't in any way be associated with me in any official capacity. (Meaning, it's not the name of a relative, or a street I lived on, or a school I went to, has nothing to do with any hobbies, or anything like that.) I keep that word and change the numeral and/or character around it when I need to change it. On my paper backup, I only keep track of the number/character change, knowing that the word is the same. So even if someone were to find my "cheat sheet", it wouldn't be of any use to them.
So far, it works. But there's talk of creating a system in our organization where the password is going to have to be completely different each time. So I'm working on coming up with a system for that.....
