This situation arises quite often where I work. We do not put it on the agenda as often it is something that has cropped up after the agenda has been circulated. The Chair will be made aware of it in advance of the meeting and will bring it up under 'any other business'. It will then be minuted as being raised under AOB and taken forward from there.
