Should a doctor's address or work address appear on their C.V. at the top? How should the educational and academic appointment dates be placed - inascending or descending order?

There is no "should" about CVs. There are lots of styles, lengths, depth of detail and ways of presenting CVs.
With respect to contact details - these really "should" be at the beginning of a CV as an interviewer will always need these to hand very promptly if he/she wishes to contact a candidate. Similarly employment and educational history generally are best listed in reverse order, starting with the most recent dates.
The most important thing is that CVs should be clear, truthful and logical, as well as presenting a candidate in the best possible manner. In general, most interviewers don't want to read a book about each candidate, so a fairly short CV is best, often no more than one page of A4, although maturer candidates and those who have considerable educational attainments may need a bit more space.
When it comes to academic or senior medical appointments, a much more detailed CV may be required, listing not only employment history but also research projects undertaken and publications they have written or contributed to, panels and task-orientated groups they have worked with and even lectures given at conferences etc.
